I infer from your reply that a setup of qmail and vmailmgr can only handle one real domain. Any domain on top of that has to be handled as a virtual domain?

I have noticed that users connecting to imap port of realdomain1.org can authenticate using only username and passwd. Users connecting to imap port of realdomain1.org and accessing mail for virtdomain1.org need to log in using user@virtdomain1.org as described in the documentation. What I don't understand is that users connecting to imap port of realdomain2.org, accessing their mail for the realdomain2.org domain have to also use the user@realdomain2.org style of authentication.

The local IP address of the connection is completely ignored. Your configuration just happens to use virtdomain1 as the default domain, and the local IP address of the connection is not used in any way.
