I am using RAV anti-virus ( http://www.ravantivirus.com/ ). It is much cheaper compared to Sophos, and it has hourly automatic pattern file update. To me, it has been working fairly well. Installation is a snap, and configuration is easy. Can do SMTP Auth etc. as well.

My site is not so high volume as to email is concerned, so I cannot talk about the scalability, though.

In the past I have used Sophos sweep for anti-virus scanning, integrated to sendmail by means of inflex. Can anyone point me at something like inflex, but which will work with Courier?
