Would it be easy to patch courier myself in order to check esmtproutes for incoming mail or make it do something equivalent? I do have C and C++ skills, but the question is: how many lines of code would be involved in such a patch approx?

For incoming mail, the check is that the domain in the return address is valid. Either it is valid, or it is not valid. esmtproutes supplies the default domain-based destination when sending mail (if the domain is found its MX records are not even looked up).

You can already set any environment variable, including BOFHCHECKDNS, based on the connecting IP address. If all you need to do is disable return address checking for connections from fixed IP addresses, just set BOFHCHECKDNS in smtpaccess.
