Any ideas where I can look for any additional errors / reason why this might be happening?

I know where I would start looking on this, include/class.support.php, the getEmailInfo method. In my dev version, you will want to start adding debug statements about line 512:

if ($info['ema_use_routing'] == 1) {

I would start off by printing all the addresses in the $addresses array, and go from there.
