I'm not exactly sure what you mean by 'first run' and 'second run,' but please let me know if the following steps work for you:

1. Navigate to http://mapwiki.googlemashups.com/ . 2. Click the pager's 'Next' link directly below the list of locations until it dims, indicating you have reached the end of the list. 3. Click the "Create" button near the upper-right corner of the map display. 4. Place your pin anywhere on the map display and enter a title in the information bubble that appears. 5. Click OK.

You should see the pin appear on the map display as well as a new item at the end of the location list. If you didn't see this before, it's probably because you had not paged to the end of the list. In this case, the pin will disappear immediately after it's created and won't appear until you page to the end of the list since the pins shown on the map display correspond directly to the entries shown in the list.
