I didn't notice the patch sitting there until you mentioned it. Why does the distribution ship with a patch rather than just merging the changes into the distribution? It also would have been useful if the readme had mentioned the patch.

Assuming you're using the p7 or p8 versions, you'll need to apply a courier-specific patch (which I recall comes bundled in the dist) before those instructions will make sense.
