I hope this can help you somewhat, because I'm doing things from an LDAP
angle and you're using MySQL, but the principle's the same. I have:

Courier-based virtual disk quotas
I do not have:
SquirrelMail quota plugin
jamm or anything like it.
My Courier-based disk quotas are implemented from maildrop. If you have
compiled your own and do './configure --help | less' you'll see that
there is an option to compile in MySQL support (just as for me there's
an option to compile in LDAP support). If you do this, you should be
able to store your disk quotas in MySQL. You'll have a
"maildropmysql.config" that you can copy to /etc to let maildrop know
how and what. Unfortunately, although LDAP support is documented in
INSTALL.html, I can't see any docs that describe the MySQL possibility.
You'll then use maildrop in postfix master.cf as transport, not in
main.cf as mailbox_command, since that is ffor local(8) delivery, whilst
you're using virtual. The new Postfix docs at ftp.porcupine.org (follow
the link from www.postfix.org) have a Postfix+maildrop HOWTO.
