Ctrl + Measure/Meter tool creates guides. Otherwise the tool does its other function - measure. I think this distinction was added to the tool with SU6.

A tool modification reminder shows up down on the status bar when the tool is selected.

You want the little '+' sign to be present, next to the measure cursor icon before doing the final click to set the guideline. It doesn't seem to matter when Ctrl is pressed, just as long as the '+' is visible before the final click.
