Three more fixes checked in under one revision (16). This one involved XSLT _and_ CSS changes.

Support output as XHTML 1.0 Strict or HTML 4.01 Strict http://code.google.com/p/gsa-xhtml-stylesheet/issues/detail?id=14

Cached Docs and PDFs do not display properly (IE6 and IE7) http://code.google.com/p/gsa-xhtml-stylesheet/issues/detail?id=17
