I've been told that the VQAdmin's .qmailadmin-limits generated file has disable_webmail (and I've verified it on one of my client's test domains), but it seems that their SQWebmail cgi is not noting that they've disabled web access.

They've told me in the past that this works, but it's not working now (verified). What authentication module do I need to activate to get SQWebmail to check this file per domain?

Since sqwebmail does not include any vqadmin-specific code, you must be referring to changes to the sqwebmail code from the vqadmin package. SqWebMail's authentication modules have nothing to do with vqadmin, and do not include any vqadmin-specific code.
