Answering my own question here... well, kind of. Turns out it wasn't an issue with the Maildir files being corrupt. Instead, it was a single message, sent from a Sendmail 8.9.3 relay that picked it up from an Exchange server, that was causing the problems in Pine. The message had well over a hundred addressees in the To: field (it's a press release ;-)), and some weird Exchange message-ID headers too.
