OK, I admit--I'm a bit sleep deprived. I just got a couple of hours of sleep, and I see now I hadn't started authdaemond. I started that, restarted sqwebmail, and tried to log in using sqwebmail. This time, the login request did get processed and logged in mail.log. However, I noted two things in particular: first, my password was logged in clear text. Second, only my first login attempt was logged in mail.log--subsequent attempts were not logged. I'm sure I'm making a stupid configuration error, but I've been looking at this for so long now, I just can't see it. Any ideas? Here are the entries for my first login attempt from mail.log:
Dec 22 22:17:32 localhost sqwebmaild: LOGIN: ip=[(null)], username=user@myisp Dec 22 22:17:32 localhost sqwebmaild: LOGIN: ip=[(null)], password=passwordincleartext Dec 22 22:17:32 localhost sqwebmaild: LOGIN FAILED, username=user@myisp, ip=[192.168.1.106]

Unless you are also running Courier-IMAP, which also uses courier-authlib, you are running some other IMAP or POP3 server which implements its own authentication, separately.
I am running Courier-IMAP and Courier-POP. Here are all the Courier services currently running on my server:
