Well, anyone sending a significant amount of mail is foolish not to run a
caching name server...

Additionally, it's clear that couriertls was originally designed to
only be used "within" courier (rather than by users), because if it
wasn't, one would probably pre-suppose that /etc/hosts would be
utilized.
What other MTA does this by default instead of calling directly?
calling what directly? You mean what other MTA doesn't use the libc's
name resolution routines? qmail doesn't. I'd be surprised if *any* other
MTA *doesn't* use the OS's name-resolution software.
