Well for me I use spamassassin and it gets about 99.9% of the spam out of about 500-600 spams a day I get maybe one every other day or so. So far there are is only one group of false positives and it is a mailing list I'm on were one of the devs is from china and my big5 rules set it off but he could be white listed easily enough.
