AUTHMODULES in /etc/courier/imapd contains the same 4 modules.

/etc/courier/authdaemonrc does not exist.

Also, what are the installed permissions on the webmail binary in the web server's cgi-bin directory?

Try using the authtest program to test that particular login. It runs the same authentication code as webmail.
