Here is the best solution I have found for VB apps http://www.icarz.com/mysql/

The DLL works great, the functionality is complete, the docs are good, and the sample VB app is both clear and useful.

On the other hand, I did take a look at gmysql.zip and found that it did not have nearly enough functionality to be usable in a real app.

And MyVBQL.dll has been LGPL'ed
