You are trying to send response headers after sending output. You will get
that error every time. Output can be a single white space, a new line,
actual content.. anything. So make sure you calls that send response headers
are always made before sending output.
And no, don't use output buffering to cover up bad code. Write your code
cleanly by calling response header sending functions before sending output.
