Why would <fmo@shiva.fmp.com> not receive a DSN on this? Is there a way to force this? I _don't_ have DSNNOTIFY=N in /etc/courierd/courierd.

Check the exit code from ezmlm. It's probably not one of the exit codes that's considered a permanent failure. This is handled as a deferall, and the mail will still be in your queue.
