It may not be a RAV issue, as long as you positively ascertain that you're sending a 500 back to the sender.

There are a small number of extremely broken mail relays that simply vomit all over themselves if they receive a 5xx error to a DATA. Instead of bouncing the message they treat it as a temporary error, and retry the message again.

The only solution here is to blacklist the sender's IP address completely, until they get their act together.
