So do you call qmail-scanner from Maildrop? If so, is there an example
of that configuration somewhere? If that is possible, that would be a
perfect solution.

I am using clam antivirus, it is free and works great.
As for calling the antivirus, I use qmail-scanner to call clam and SA.
