The webserver user doesn't, but whatever user the mail runs as will need access.

Yes. In the /path-to-eventum/include/class.support.php and class.note.php there is a method called "saveRoutedEmail". You can change the directory the messages are saved too.

In the method I mentioned above, You can disable saving the message. You can also just change the permissions on the routed_emails and routed_notes directories and it will skip saving the messages.
