For DocBook V5, does our "http://docbook.org/ns/docbook" namespace conform to these guidelines? Is this namespace an approved exception?

These guidelines were not pubslihed when we voted for this namespace. There are many OASIS namespaces in use that are not conforming to this rule. See http://lists.oasis-open.org/archives/docbook-tc/200503/msg00000.html

So I don't see any possible problem here.
