Try using 'makeuserdb' instead. It should have the correct path to makedatprog linked into it.

On my FreeBSD-5.3 box;

# grep makedatprog /usr/local/sbin/makeuserdb $makedat="/usr/local/libexec/courier-authlib/makedatprog";
