Thanks to Marty and Hima for many detailed comments on versions 1.05 and
1.04, respectively. I propose to accept further editorial comments against
version 1.05 (as well as any responses to Marty and Hima) through Friday. I
will then address outstanding editorial comments -- before referenced line
numbers change -- in version 1.06 to be distributed shortly thereafter. I
would like to have technical contributions resume for version 1.07.
