I am using it on an Outlook 2003 and Exchange 2003 server setup and it works fine.

Yes, it's possible. Just create an email account, and enable the "auto-create issue" feature. You will have to setup a cron job to run every so often, and the PHP script will download new emails available on a POP3/IMAP mailbox, and automatically create new issues out of them.
