Yeah I've wondered the same thing, I've never received a CAPTCHA challenge
either. Is there a rate at which CAPTCHAs are issued? Does it have to do
with certain factors like the frequency of logging in or the age of the
account? Or is it possibly something that hasn't been implemented yet?

I've tried logging in with an incorrect password up to 20 times in
quick succession, but still I only get the normal "BadAuthentication"
error, and an attempt to login with the correct password following
this will always succeed without any challenge.
Any tips?
