I don't use courierfax, so I can't help you with how it should work, but can you confirm that you have the correct syntax in your smtpaccess file. It should say:

192.168.135.248<TAB>allow,RELAYCLIENT,FAXRELAYCLIENT

Then run makesmtpaccess again and see if that helps.
