Why not just have the app rescan all the folders every X minutes? No mods to any server software required.

Both sylpheed (gtk+ based) and squirrelmail (webmail) do this very well. sylpheed makes good use of its imap cache, making for very fast rescans. squirrelmail is somewhat slower, but still nice.
