Is it possible to set up Courier so that authenticated SMTP is required for everyone except one particular ip address?

yes, add that address to /etc/courier/smtpaccess/default according to the examples that you see there.

The examples and man pages for smtpaccess don't mention anything about authentication. I can block the ip, or allow it to relay. What setting would I use to allow it to connect without authentication?
