Yes, I restarted courier-mta after the update is done, I also set the MAXDELS=400000 in the module.esmtp file.

The connection refused error was reported by the smtp server performance testing tool (http://www.icewarp.com/download/tools/srvtest.zip), when I saw the error, I ps the courieresmtpd processes which the total number is about 40, this is because I set the option "maximum concurrent threads" to 40 in the testing tool, now I increased it to 100, I can see more courieresmtpd process is created, however, I saw the connection refused error again when the couriersmtpd processes are around 60,

22:05:08 [10061]: No connection could be made because the target machine actively refused it

Did you restart Courier, after making the changes in the configuration?

Furthermore, you should not get 'connection refused' in any case. If you reached MAXDAEMONS, the connection should be made, but there will be no response until one of the existing connections drops off. If you reached MAXPERC or MAXPERIP, the connection should still be made, then you'll be immediately disconnected.
