Can you reproduce this easily? Can you explain how you get it to fail?

Easily in the sense of we don't have to do any work for it to happen. It was
mainly failing after a seemingly random period of time. In some cases this was
right after boot, in others it would range anywhere from 12 to 36 hours after
boot.
