Does anyone have a recommendation for a mac or unix imap client that works with courier to make use of top level folders?

Any modern client will display the IMAP structure however you tell it to. So in Thunderbird go into the properties for that account and choose Server Settings->Advanced Button->IMAP Tab and put "INBOX." into the "IMAP Server Directory" field. Then all your folders will appear as sisters of the INBOX.

You can do the same in Outlook and Evolution. Hunt around until you find the right place to enter "INBOX." as the "Server Directory" or "IMAP Root" or whatever they call this setting.
